You get the option to do so when you put an Ash of War on it. You might need the specific Whetblades to get access to some of the modifiers. (Edit: For example, if you put a Heavy Ash of War like Ground Slam on something, you have the choice between Heavy and Normal - unless you have Iron Whetblade, then you can choose Keen (aka Sharp) or Quality, too.)

You have to change the ash of war on the weapon, when you pick a new ash of war then another option pops up where you can pick heavy / fire etc

They are tied into the ash of war you equip onto the weapon, but some weapons have a unique ash of war and can't be switched, but most weapons you find can be changed.
Each ash of war also tells you what type it changes the weapon to, somewhere close to the top left, i would tell you the exact location, but I'm at work right now.

I have a lot of smithing stones but when I walk up to the blacksmith, try to upgrade any weapon it says “additional materials required” but it doesn’t really tell me what other materials I need. Help?
It tells you what you're missing. A smithing stone (3) doesn't mean it's a stack of 3, it means it's tier 3.
You need 12 of each smithing stone to reach the next smithing stone tier till +24 then you need 1 ancient smithing stone
Somber stones are more simple only needing 1 each tier till +9 then you need 1 ancient somber smithing stone.

Depending on the level and the kind of the weapon it requires a different amount of a specific Smithing stone, as well as runes
Make sure you have the right smithing stones, each smithing stone has a number that indicates what upgrade range it can upgrade, e.g. smithing stone [1] can upgrade a weapon from +0 to +3, smithing stone [2] then upgrades a weapon from +3 to +6 and so on and so forth.
Certain weapons require somber smithing stones where a different stone is needed for each upgrade level.
Also, weapon upgrades cost a small amount of runes, it's usually only a few hundred.

After this and once you have access to the round table hold, there's an NPC there that can upgrade weapons all the way up

There's an anvil in the church just north of where you started. You can upgrade weapons up to +3 there if you have enough stones. To go higher than that, you'll need help from a blacksmith. Just keep exploring, and Melina will eventually hook you up.

As far as I can tell, yes. I can't confirm, it could be just luck, but I've had better rings/amulets (+2 or +3) on Veteran difficulty. I saw none on Easy, just regular all the way.

But I've mostly been trying to do my 1st playthrough blind, and assumed mats were similar to Bloodborne etc. where you generally have to pick a weapon or 2 and dump everything into that.
I've been using spells a lot though, and found a few upgraded weapons in the world (some +8 ones from somewhere).
You either aren't close to finishing the game or you've been strolling through endgame areas with +8 normal weapons, which... like I know you might not wanna decide on a weapon in the early game or whatever, but at some point the normal enemies take 5+ hits to kill and it becomes very obvious that you need to upgrade.
Going through the game blind means you have to be at peace with making some mistakes and trusting the game that they won't be permanent, or that you can still have a good time with suboptimal setups.
Oh well. I do hope you're in something like Leyndell and it's not actually the endgame for you.
Also there’s about 7 Somber +10s and maybe 8 +25 Stones in a single playthrough depending on how many quests you’ve done so as long as you like your build or a certain few weapons you should have plenty

Up to +24 for smithing stone weapons and up to +9 for somber weapons. It's only the last upgrade that's a limited material. Everything else is infinite with bell bearings.
You should really upgrade the weapons you've been using. You've been playing at a huge disadvantage if you haven't upgraded any of your weapons. Unless you want to continue this as a challenge.

I'm new to souls like games so I'm not very familiar with the conventions. I have some questions about upgrading weapons since there's not any in game directions on this topic.
Weapon upgrades are very impactful, one upgrade could give you an extra 100 damage; if you mean should you focus on one weapon or multiple I'd say one because the stones are hard to get early on; the area you get the weapon doesn't matter, there were multiple weapons in the starting area that I used till the final areas and some late game weapons I found to be bad, so it all depends in the weapon itself; you can infinitely buy ever type of stone by the late game but until you do that they are very hard to find
I’ve been farming 1 stones @ the soldier camp in the ruins next to the Gatehouse in Limgrave — just got two greatswords to +3, dual wielding is so much fun.. it’s been taking a while to accumulate ‘em but it’s been pretty easy overall & I’ve also snagged a full set of Godrick Knight armor from the random drops there

Just finished my first playthrough. I was looking for a way to upgrade my jewelry the entire game but never found one. Did I overlook it? Most of the jewelry I found throughout the game was abysmally bad for 2 handed weapon builds. Like +3 health or 1% crit. I ended up using the +25 encumbrance ring for the entire game. don't know if the devs have this planned or I missed it but it felt strange not being able to upgrade them.

So the way it works in Wilds is a bit different to World. Going across in the same row is upgrading but going diagonally across actually crafts a whole new weapon which you can then upgrade along that row.
To take dual blades for example, crafting the bone hatchets allow you to then build the dosha Fastflayers but will not get rid of the original bone hatchets.
To upgrade the Dosha Fastflayers, you will need to progress further into the story until you gain access to all the materials required to upgrade them.
I just reached the oil basin and there you can upgrade your armour, but the weapon I think it's the tree thing

The tree can be really annoying when you're still learning his movements. My advice: don't stick too close. Let him stand up and fall on you, and dodge to his left / your right, or let him charge and dodge it (more difficult but doable), because after these attacks he has a nice window for some hits.
Just keep playing my man, you only have to worry about farming them towards the end if you want to try multiple builds in one playthrough, you should get more than enough to max out 2 - 3 normal weapons, and 1/2 boss/special weapons just through normal gameplay as long as you explore thoroughly and kill everything.
+1/+2 is pretty much standard for fighting the curse rotten greatwood
